Patient Requirements

To be seen at the Matthews Health Clinic, you must:

1. Live in Mecklenburg or Union County
2. Be 5-64 years old
3. Not have Medicaid, Medicare, VA or private insurance
4. Attend an eligibility screening held on the second or fourth Mondays of each month between the hours of 9:30 am and 11:45 am. A volunteer will review the application. Attendees will be notified shortly after if they qualify. Doctor appointments are given to qualified patients only.

What to bring with you

Bring a completed application with an original and one copy of the following items:

1. Picture ID
2. Social Security card, if available
3. 2 consecutive Payroll stubs (most recent) or letter from employer
4. Utility bills with your name on it
5. Mortgage statement
6. 2 consecutive bank statements
7. Current year's tax return
8. Disability statement, if applicable
9. Food stamp letter, if applicable
10. Unemployment benefits letter, if applicable
11. Child Support, if applicable

     Copies will not be made at the clinic.

Recertification

• MHC recertifies patients every 12 months from their starting certification date.
• You must bring your current year's tax return, most recent payroll stub, recent utility bill, and disability statement, if applicable.
